VIC – Victims of Crime Tribunal – Oaths and Affirmations Act 2018

As a result of the Oaths and Affirmations Act 2018 which came into effect on 1 March 2019, the following forms have been updated :-

  • Application for Assistance [LL-VIC-VOCAT‌-001]
  • Statutory declaration [LL-VIC-VOCAT‌-011]

We have also been advised that the transition period (which ends on 31 December 2019) only applies to Statutory Declarations and not to Affidavits.

VIC – New Personal Injury and Worker’s Compensation forms under the Wrongs Act 1958 (Vic)

The following new forms under the Wrongs Act 1958 (Vic) have been added to Personal Injury and Worker’s Compensation matter types:

  • Form 1 – Certificate of assessment of degree of impairment arising from stabilised injury (LL-VIC-WRA-000)
  • Form 2 – Certificate of assessment of degree of impairment where injury has not stabilised (LL-VIC-WRA-001)
  • Form 3 –  Agreement to waive assessment of impairment (LL-VIC-WRA-002)
  • Form 4 – Claimant prescribed information form (LL-VIC-WRA-003)
  • Form 5 – Notice of referral of medical question to medical panels (LL-VIC-WRA-004)

Vic – Victims of Crime Assistance Tribunal Forms

Further to our blog earlier today regarding Form 1 being updated, we have now completed our review of the Victims of Crime Assistance Tribunal Forms.

The ‘Application for extension of time’ (LL-VIC-VOCAT-005) form has been deleted as it was revoked in accordance with Practice Rule 2 of 2016.  The extension of time for lodgement is now captured within Form 1.

All other forms were updated in accordance with the latest version.

Vic-Victims of Crime Assistance Tribunal Form 1

The Application for Assistance Form 1 Rule 6 (Form 1 – LL-VIC-VOCAT-001) has been updated in response to client feedback.

We will undertake a complete review of the Victims of Crime Assistance Tribunal forms and advise when this has been done.

FED – Medicare Forms

A review of the Medicare and Centrelink forms has been undertaken and the following 2 forms have been updated in accordance with the Australian Government Department of Human Services website –

  • Medicare Australia – Notice of Judgment or Settlement (no bar code) (LL-FED-GEN-012); and
  • Medicare Australia – Section 23A statement and Statutory Declaration (LL-FED-GEN-018)

All other forms are current.
