  • Imminently fatal asbestos-related disease worker claim – Form 1 – LL-ACT-MC-217
  • Imminently fatal asbestos-related disease medical certificate – Form 1A – LL-ACT-MC-218
  • Imminently fatal asbestos-related disease dependent claim – Form 2 – LL-ACT-MC-216

The following New Default Insurance Fund forms have been added:

  • Approved medical certificate for ACT workers compensation claims LL-ACT-MC‌-228
  • Employee’s claim form LL-ACT-MC‌-226

ACT Workers Compensation Act 1951 – Default Insurance Fund

The following new forms have been added to ACT Personal Injury, Workers Compensation, LEAP Forms,  Default Insurance Fund, profile:-

  • Imminently fatal asbestos-related disease medical certificate – Form 1A – LL-ACT-MC218
  • Imminently fatal asbestos-related disease dependant claim – Form 2 – LL-ACT-MC-216

FED – Medicare Forms

A review of the Medicare and Centrelink forms has been undertaken and the following 2 forms have been updated in accordance with the Australian Government Department of Human Services website –

  • Medicare Australia – Notice of Judgment or Settlement (no bar code) (LL-FED-GEN-012); and
  • Medicare Australia – Section 23A statement and Statutory Declaration (LL-FED-GEN-018)

All other forms are current.

  • Statement of claim – death or personal injury other than motor vehicle or employment-related – Form 2.5 – LL-ACT-MC-0005;
  • Application for arbitration by injured worker – Form 3.57  – LL-ACT-MC-0154;

ACT –  Personal Injury – Supreme Court

  • Statement of claim – employment death or personal injury – LL-ACT-SC-0027;
  • Statement of claim – death or personal injury other than motor vehicle or employment related – LL-ACT-SC-0028.
